The gaps people miss
- The form's wording covers the gaming floors of the 4 racetrack casinos plus The Greenbrier. Online coverage comes through rule 179-10-2.38, which treats anyone on a self-exclusion or casino/Commission exclusion list as a prohibited interactive gaming participant.
- Limited Video Lottery machines in bars and restaurants have NO statewide ban (Problem Gamblers Help Network).
- Traditional lottery tickets are not covered.
- There is a one-year minimum, then removal needs a written request. It is not a fixed term that ends on its own.
- Operator-level 'suspensions' (72 hours to 1 year) are not the statewide list.
- Prediction markets (Kalshi etc.) and offshore sites are not covered.
- Entering a WV casino while excluded is trespass, with possible prosecution.

- Get the WV Lottery Self-Exclusion Request Form from wvlottery.com (iPlay with Purpose > Self-Exclusion) or from the Problem Gamblers Help Network (1800gambler.net/self-exclusion).
- Fill it out and attach a copy of your photo ID.
- Either complete it in person with WV Lottery security personnel, or have it notarized and mail it to West Virginia Lottery, 900 Pennsylvania Avenue, Charleston, WV 25302.
- Self-exclude inside every sportsbook/online casino app you use right away. Online accounts are tied to the casino licensees' exclusion lists.
- Limited Video Lottery (bar/restaurant machines) have no statewide ban. Ask each venue, or avoid them.
What you need
Copy of photo ID attached to the form. Notarization is required if mailed. The form also asks for physical description, SSN and driver's license details (per a secondary guide).

What happens to your balance and open bets
Do this first, whatever your state says: withdraw your balance and let open bets settle before you enroll. Getting money back afterward means dealing with an operator that is now required to refuse you.
Per rule 179-10-6, online accounts of prohibited participants are suspended: no wagers or deposits, and withdrawals are blocked unless the suspension was patron-initiated. Pending wagers are cancelled or refunded under the operator's internal controls. Operators' WV terms (theScore Bet) say winnings are forfeited if an excluded person plays. individual operator payout of balances.
Can you get off the list early?
No removal in the first year. After one year you may send a written removal request, which the Lottery 'will consider' (not automatic). Otherwise the exclusion stays in place indefinitely. the current removal form.
